A Tulsa, Oklahoma Oral Surgeon was investigated by the State Dental Board, State Bureau of Narcotics and the Federal Drug Enforcement Agency after a patient tested positive to HIV and Hepatitis C without any known risk factors other than receiving dental treatment.
After a surprise inspection, inspectors found the following problems in the dental office:

Regular use of rusty, porous instruments that cannot be properly sterilized

Practice of pouring bleach on wounds until they turned white

Many violations in sanitary practices, such as contaminated equipment

Allowing unlicensed dental assistants to administer medications

Drug cabinets were kept unlocked and unsupervised

Inventory drug logs were not maintained

Use of expired drug vials

Use of needles and drug vials more than once
